How we work together
Here in West Lothian CHCP we have a simple ethos - 'how can we make life better for people?' To make it better tomorrow we acknowledge that something has to be done today. This preventative/proactive approach is reflected in many of our current projects. However, we have discovered that it is the way we weave the services together that really makes a difference for people. Very rarely does one service meet all needs; therefore it has become our practice to offer a range of services that complement each other. This is not a scatter-gun approach to service delivery; we see the person and consider what we could offer that would really make a difference, adopting a whole person approach wherever possible. With the inception of the CHCP, we are in a better strategic position to draw local services together, having both Primary Care and Social Work under one Director, fortnightly meetings of the senior team, allied with the broader extended management team focusing on projects that can draw on the combined resources of both organisations. The CHCP has also made extensive use of new technologies to support innovation in service planning and delivery to bring services closer to service users and also to support the work of CHCP staff. Some of the initiatives we have developed and enhanced during the life of the CHCP include the Home Safety Service, Body Checkout, eCare/C-Me electronic shared information systems, delayed discharge, long-term conditions, Books on Prescription and the Exercise Referral scheme.
