Service co-ordination team

Service co-ordination staff provides an assessment service to identify support needs for those individuals who, due to addiction and mental health difficulties, require assistance to meet their tenancy conditions.  

Service co-ordination personnel will undertake an assessment of support needs and thereafter arrange appropriate Agencies to provide that support.  

The support put in place will have an enabling function to build on skills, increase confidence and develop abilities to facilitate independence in these areas.Service Coordination staff are based at Strathbrock Partnership Centre in Broxburn.

The service is available for those between 18 and 65, living in their own tenancies in West Lothian and who have mental health / addiction difficulties and who need support to manage and organise their home.

Support available:

Life skill training
? Support to undertake essential tasks related to maintaining their tenure, including shopping, cleaning, domestic tasks, reporting repairs and dealing with correspondence, the intended outcome being to building up confidence and independence in fulfilling these tasks.
? Support with budgeting, bill paying and benefit claims.

Socialisation
? Support in referring to providers of specialist advice e.g Welfare Rights, advocacy services addiction services and other community groups.
? Support in engaging with statutory and voluntary groups.
? Support to develop links with local community for leisure, social or educational needs.

Health and Wellbeing
? Provide emotional support or directing to specialist services e.g counselling.
? Encouragement to monitor health by accessing specialist services, healthy eating, smoking cessation etc.
? Support to comply with treatments e.g taking medication, attending appointments and complying with medical programmes.
? Support  to engage in behaviour management programmes.
? Support in complying with drug / alcohol treatment and testing

Personal safety and security
? Establish security routines e.g safe use of and maintenance of appliances, house security etc.
? Encourage to maintain and take responsibility for their personal safety in and out of the home.

The aim of the service is:
- To link into service providers and community agencies
- To provide information, support and representation by arranging services from appropriate agencies.
- To help boost confidence/self esteem, self-awareness and independence
- To provide new opportunities by using existing community resources
