OSCAR
(Optimal Summarising, Coding and Accurate Records) The OSCAR project commenced in West Lothian in 2002. The main aim of the project was to support all GP practices in West Lothian to develop comprehensive clinical databases using a standardised coding formulary and protocol. At initiation of the project major problems were identified in the summarising of clinical records in primary care: - Inconsistency of diagnostic coding
- Widely varying completeness across practices
With advances in medical management and Information Technology, it is imperative that practices have accurate, up to date clinical databases. In particular in relation to ensuring more efficient identification, tracking and treatment of patients with long term conditions, maximising the care they receive in primary care and reducing unplanned repeat medical admissions. Project success has been demonstrated by: - Recognition of the success of the project and utilisation of project resources across Scotland
- Annual audits demonstrating improving standards

- To open the OSCAR coding formulary (alphabetically) and (chapters), right click on the name, save target onto desk top, right click the icon on the desktop and choose 'open with'. Open with microsoft excel.
