Learning disability service
In line with the recommendations of 'The Same As You' review of services for people with learning disabilities (Scottish Executive 2000) we are taking the clearly stated wish of adults with learning disabilities very seriously. Their wish is - they want to live ordinary lives in their own homes. We all know how important it is to have somewhere to call home, something that in the past has been difficult for adults with a learning disability to achieve. West Lothian's Plans aim to change this. For adults already receiving support and for those who would like to explore new options it will be possible, if they wish, to work towards independent living through a series of carefully supported stages. Changes to our service - Re-provisioning of Deans House, leading to a brand new care home. This will provide support to enable people to move on to greater independence
- Expansion of Burnside respite unit, giving people a greater sense of independence within a supported environment
- An expanded, specialist, housing support service giving more adults the opportunity to be supported in their own tenancy

Following your referral to the Service and before your placement begins: - We will offer you the opportunity to visit and discuss the services available to you.
- We will offer you the opportunity to choose from the services that are available to you. Once your placement is confirmed we will send you a letter from the Service identified advising you of:
- Your start date.
- Your named key worker.
- Your travel arrangements (where appropriate).
- Your initial programme when your placement begins.
- We will also send you a copy of your written agreement advising you of the level of service we will provide. When your placement starts: Your named key worker:
- will meet you within 5 minutes on your arrival on your first day.
- will make sure you become familiar with the venue.
- will introduce you to other staff and service users.
- We will provide you with an initial program in a format that you understand based on your assessment and your choices.
- We will involve you in a review of your initial program and care plan after 4 weeks.
Throughout your placement: - We will treat you fairly and with respect.
- We will ensure that your dignity and right to take risks and make choices will be respected at all times.
- We will support and encourage you to be as independent as possible whilst taking part in your agreed programme.
- We will ensure that the appropriate level of assistance will be given to ensure that your personal care needs are met.
- We will consult and involve you in any changes to your programme or care plan.
- We will involve you in an initial review of your care plan and the services provided to you after 3 months.
- We will involve you in a review of your care plan and the services provided to you at least once a year.
- We will offer you the necessary support to voice any concerns you may have about the service being provided to you. When it is time for you to move on from the Service:
- We will invite you to attend an end of placement review meeting.
- We will offer you the opportunity to express your views about your placement.
- We will offer you assistance with your future plans prior to the end of your placement.
