Fast Track Referral Form(364KB)
Generic Referral Form for Stroke Unit / Astley Ainslie Hospital Referring Agents. As a comprehensive assessment has been completed by these agencies the clients will not require a further screening assessment and will be added directly on to the appropriate therapy waiting lists.
Bridging The Gap Referral Form(20.0KB)
Referral Form for clients being discharged from hospital i.e. Stroke Unit who may benefit from a brief intervention by our Rehab. Assistants (under supervision)to look at small equipment issues until they are picked up by our Therapists.
CRABIS Mild Head Injury Leaflet(400KB)
Leaflet normally distributed once someone has attended hospital and been diagnosed with a mild head injury. Advises clients what to do / how to contact our Service if they are still having medical symptoms / problems.
