How we work together
The CHCP aims to establish and develop high quality local services - with health services, social services and voluntary organisations, working closely together to provide services which meet the needs of people living in West Lothian. Objectives: Extend choices and alternatives to patients and carers Reduce waiting times for assessment, diagnosis, treatment and care Reduce the number of people admitted to hospital in an emergency Reduce the number of people delayed in hospital following treatment. West Lothian’s CHCP is jointly managed by, and accountable to, Lothian NHS Board and West Lothian Council.
