West Lothian Community Rehabilitation and Brain Injury Service recently received its 1000th referral.
Since its launch in January 2000, the service has provided multi-disciplinary rehabilitation to 1,000 West Lothian residents.
In order to maximise clients’ skills, functioning and level of independence, the service rehabilitates people within their homes. The team delivering the service includes physiotherapists, occupational therapists, speech and language therapists, and clinical psychologists. The service is also part of the West Lothian Community Health and Care Partnership (CHCP). The team works with clients to improve their mobility, ability to communication and perform general daily activities such as personal care or household tasks. The service also helps to improve the memory of clients and address any emotional needs or issues. Referrals to the service would be people aged16 and over with complex rehabilitation needs, who are either being discharged home from hospital or are already living in the community. Professionals such as social Workers, GPs and therapists, would usually refer a person to the service. Convener of West Lothian Council’s Health and Care Committee, Councillor Bruce Ferrie, said: “The West Lothian Community Rehabilitation and Brain Injury Service is a great example of the West Lothian CHCP delivering effective, joint-up services to people with complex and varying needs.
