West Lothian IJB Strategic Plan 2016-26

The IJB's Strategic Plan 2016-26 is at the heart of integration. The plan outlines the IJB's vision for health and social care services for the people of West Lothian, what the IJB's priorities are and how the IJB will build on a foundation of strong partnership working to deliver them.

It sets out how services will be delivered in a more integrated way to improve the quality of support for people who need them and deliver the national health and wellbeing outcomes.

The plan builds upon joint planning foundations established through Community Planning and the West Lothian Health and Social Care Partnership (HSCP), under which joint strategies and plans have been developed and are being implemented for a range of services and client groups.

The West Lothian HSCP has endeavoured to let those at the heart of communities have their say in shaping and influencing the plan. A Strategic Needs Assessment has identified health and social care needs in each of West Lothian's two localities, East and West. The data is informing the development of locality focused plans. In addition, detailed Strategic Needs Assessments are informing Commissioning Plans for five main care groups: alcohol and drugs, physical disability, learning disability, mental health and older people.
